How do I add my child under the age of 14?
If you currently have an account, you can request access by logging in and clicking on ‘Manage Account’ from the dropdown near your name and then clicking ‘Request Access to My Child’ on the left side panel. You will then need to fill out the form with your child’s information and click submit. NOTE: The child must have a SSN on record and patient and guardian's address must match. If you do not have an account, please contact My Union Health Portal Helpdesk at 812-238-4911.


How do I add my child 14 - 17 years old?
My Union Health Portal does not provide access to online medical information for parents and guardians of teenagers between the ages of 14 and 17. However, the patient may create an account and share their access with others.


I do not remember my username
Click the "Forgot Username" link on the login screen to walk through the process of recovering your username. If you do not remember the responses to your security questions, you can recover your Username by contacting My Union Health Portal Helpdesk at 812-238-4911.


I do not remember my password
Click the "Forgot Password" link on the login screen and enter your username to receive an email with a link to reset your password. You may also contact My Union Health Portal Helpdesk at 812-238-4911.

How do I allow someone to access my information?

Click on ‘Manage Account’ from the dropdown near your name and then click 'Invite Someone to Have Access' on the left side panel. You will then enter the person's email address and a four-digit activation code and click Submit. The proxy will then receive an email invitation to view your record. They will need to either create a new account that will only allow them to see your record or sign into their existing My Union Health Portal account to switch between their record and yours.
